GP Management Plans - Maximising your Health and Minimising the Cost


At Next Practice Victoria Park we are passionate about the proactive management of chronic disease.

Since 2005 the Federal Government has been encouraging Australians with chronic or terminal medical conditions to maximise their health through the use of chronic disease management plans. These plans allow GPs to co-ordinate the care of Medicare eligible patients who have a condition that will last more than six months. Common conditions include diabetes, heart disease, asthma, sleep apnoea, fatty liver, but essentially any long-lasting disease that affects your quality of life is eligible.

The benefit of these plans is that the GP can refer you to two or more allied health practitioners (eg dietitian, exercise physiologist, physiotherapist, speech therapist etc) to assist in the management of the condition and you will receive a Medicare rebate for five of these visits. Some practitioners will elect to bulk-bill and some will charge a small co-payment. These practitioners can be ideal for losing weight, education on your disease and suggestions to the GP for improved management.

The plan will also include follow up arrangements, recalls from the practice and monitoring of any blood tests or physical readings with your GP. At Next Practice Victoria Park we are passionate about the proactive management of chronic disease, so we will bulk-bill all patients where the consultation is for the preparation of the plan or where we have recalled you for review of the plan.
