According to the Australian Department of Health, those aged 45-49 are most at risk of developing a chronic illness.

If you’re 45-49 years old, you’ve reached an important time in your life where you can identify health issues early and get support to prevent or delay your risk of chronic disease.

What should you expect in a Health Assessment appointment?

  • History and lifestyle
    • Your lifestyle plays an important part in your risk of chronic disease. We’ll discuss factors such as your diet, physical activity, if you smoke or drink alcohol and maintaining a healthy weight.
    • We’ll also review your medical and family history for a variety of illnesses, such as diabetes, stroke, heart disease, osteoporosis.
  • Risk assessment
    • Your doctor will use this information to assess your risk of developing a chronic illness.
  • Make a plan
    • If needed, we will work with you to create a plan for improving your risk factors for chronic disease and keep track of your health.
