- Your lifestyle plays an important part in your risk of chronic disease. We’ll discuss factors such as your diet, physical activity, if you smoke or drink alcohol and maintaining a healthy weight.
- We’ll also review your medical and family history for a variety of illnesses, such as diabetes, stroke, heart disease, osteoporosis.
- Risk assessment
- Your doctor will use this information to assess your risk of developing a chronic illness.
- Make a plan
- If needed, we will work with you to create a plan for improving your risk factors for chronic disease and keep track of your health.
